  • Healthy Siouxland Initiative History
    The Siouxland District Health Department initiated the Assessment Protocol for Excellence in Public Health or APEXph in 1993 to assess our organization. In an effort to transition this process into the community, the Siouxland Community Health Needs Assessment Committee was organized in early 1998. Comprised of key community health care providers, the group set out to create a process to assess our community’s health status and to develop a community plan to meet the needs of the residents of Woodbury County.

    In July of 1998, the committee developed the Woodbury County Health Assessment, which was a survey that was randomly sent to residents of Woodbury County. The survey had a 41% return rate and was used to complete our community health needs assessment.

    The Siouxland Community Needs Assessment Committee transformed into Healthy Siouxland Initiative in the fall of 1998. HSI is now comprised of a broad community representation representing a variety of organizations including health and human service providers, educators, law enforcement officials, and clergy.

    HSI completed a comprehensive community assessment in March of 2000. The Health Report Card is a compilation of information and data for Woodbury County. Following completion of the Health Report Card, a three-year community strategic plan was created. Future HSI efforts are focusing on further developing the strategic plan and its implementation.

    Health Problem Profile
    A health problem is a situation or condition of people, which is undesirable and is likely to exist in the future as death, disease, or disability.

    March 2000 Health Report Card Highlights
    HSI, with the support of the Siouxland District Health Department, completed a comprehensive community assessment for Woodbury County, Iowa in March of 2000.
     
    The Health Report Card is a compilation of information and data for Woodbury County. Following completion of the Health Report Card, a three-year community strategic plan was created. The following health issues were recognized as concerns across all age grouping in Woodbury County.
     
    They are:
    • untreated dental caries
    • mental illness
    • obesity
    • substance abuse (includes alcohol and tobacco)
    • violence
    • poverty
    • nutritional issues
     
    HSI used the information gained through this assessment to formulate a strategic plan. HSI members will partner with community providers and agencies to support, enhance or create programs to address the problems identified for all of Woodbury County.

    1. Culturally appropriate educational resources
    2. Oral health
    3. Obesity in adults and children
    4. Teen pregnancy
    5. Infant mortality
    6. Cancer incidence and mortality
    7. Heart disease
    8. Diabetes

    10. Hepatitis C
    11. Domestic abuse
    12. Child abuse
    13. Sexual assault
    14. Juvenile crime
    15. Substance abuse (alcohol, drugs, and tobacco)

    In March of 2002, HSI released the Health Report Card Update. The 2002 report contains updated data on Woodbury County demographics as well as the main focus group areas. The final portion of the report provides detailed information related to the individual plans. Information is also provided on how Woodbury County's plan links to Healthy Iowans 2010 and Healthy People 2010.

    HSI Update for 2002
    Following meetings during the summer of 2001, in addition to working on the issues identified in the Health Report Card, HSI realized a need to clarify what the purpose of HSI was, and how HSI could work together to affect policy on the local, state and national level.
     
    The group worked on developing a position statement and guidelines for supporting issues. The agenda for the monthly meetings was also modified to reflect the future direction of the group.
     
    Purpose Statements Funding - To heighten awareness of private and public resources to support initiative targeted to improve the health status of the community.
     
    Reporting - To collect, analyze, and report data that is relevant to the health status of the community.
     
    Supporting - To lend our collective voice in support of grants, regulations, ordinances, policies, and legislation that will improve the health status of the community.
     
    Collaborating - To bring consumers and/or providers together for the expressed purpose of coordinating services and maximizing resources to improve the health status of the community.
     
    Planning - To create a community plan that reflects the needs of all populations and correlates with state and federal health plans such as Healthy People 2010 and Healthy Iowans 2010.
     
    Educating - To increase the knowledge of consumers and providers about matters relevant to the health status of the community.
     
    HSI continues to meet monthly as a large group with individual focus groups working on the top health problem priorities as defined in the March of 2000 Health Report Card.
     
    Those health problem priorities are:
    • teen pregnancy
    • infant mortality
    • substance abuse
    • oral health
    • hepatitis C
    • domestic abuse
    • child abuse
    • sexual assault
